Northwest Renal Network (ESRD Network 16) strives to promote optimal care for individuals with End-Stage Renal Disease.   Optimal care not only includes safe and quality care, but must also include efforts to improve patient independence, quality of life, and rehabilitation.   The new CMS Conditions for Coverage (CfC) for ESRD Facilities place even greater emphasis on these issues.   Therefore, the Network conducts focused activities such as the annual vocational survey to assist renal providers in improving vocational rehabilitation.

The 2007 Vocational Rehabilitation Toolkit

We've preserved the 2007 Toolkit in case there was a resource on it you still use.   The 2007 Tookit was the first version created by the Network.   In the Toolkit, the Network provides several new resources to facilities including a best practice checklist for facility operations and patient care, a summary list of resources, and a summary report of Network vocational status.   In addition to these new resources, the toolkit contains copies of vocational resource and patient educational materials from a number of community and governmental agencies.

A tailored version of the 2007 Toolkit was sent to Washington State facilities.   In past communications with the Network, facilities in Washington State had expressed frustration in accessing the Washington State DSHS Division of Vocational Rehabilitation (DVR).   In response, the Network collaborated with DVR and created a version of the Toolkit with Washington-specific resources and information.
