CMS Compliance standards for the 2728 and 2746 Forms

 

 

Northwest Renal Network is responsible for tracking facility performance in timely submission and accuracy of the 2728 and 2746 forms under the terms of our contract with the Centers for Medicare & Medicaid Services (CMS). A federal database system used by all Networks calculates accuracy and timeliness.  Northwest Renal Network generates two reports a year from this system:  the semi-annual and annual compliance report.  These reports are sent to individual facilities as well as the CMS Region X office.

 

The standard set by CMS is for each facility to maintain an average compliance rate of 90% for timeliness and accuracy. The definitions of “timeliness” and “accuracy” are established by CMS.  

 

The 2728 form (Chronic Renal Disease Medical Evidence Report) is to be submitted within 45 days of the event that triggers the form (e.g. the date of the first chronic dialysis at your facility or the date of a transplant).

 

The 2746 form (ESRD Death Notification) is due within 30 days of the patient’s date of death.

 

We understand that under some circumstances the 2728 or 2746 form may be delayed beyond the time limit set by CMS; however, the Compliance Report standards are based on these time limits. 

 

Network staff will be glad to provide technical assistance or answer any questions you may have to help your facility achieve and maintain an acceptable compliance rate.  We also welcome visits to the Network office and can arrange on-site or teleconference training.

 

For those facilities falling below the CMS goal of 90%, it is expected that the data coordinator along with the facility manager or administrator will develop an Action plan for improvement.  You may use our Sample Action Plan, or you can submit the same information in another format.
