The Network works with Data Coordinators at each dialysis facility to monitor ESRD patient status and to maintain the completeness, validity and accuracy of the CMS ESRD Forms and Network Reports.

CROWNWeb

CROWNWeb is a CMS-mandated data collection system that all facilities will be required to use upon national implementation.    Currently, CMS has targeted Spring 2012 as its goal for national implementation.   In order to have access to CROWNWeb, each ESRD Network 16 facility needs a minimum of one Security Official and one QIMS End User Manager.   Terms are defined, and additional instructions provided, in CROWNWeb/QIMS Common Terms and Other Resources.   QIMS training videos are also available on the Project CROWNWeb website at http://www.projectcrownweb.org/crown/index.php?page=CROWNWeb_Training&subPage=Training_Materials.

Facilities are required to notify the Network when a patient becomes lost to follow-up, by completing a copy of the Lost to Follow-Up Checklist and faxing it to the Network at (206) 923-0716.   It is against federal law to send identifying or personal health information via email.
