HIPAA Privacy and Security Toolkit

2008 Vocational Rehabilitation Toolkit

CROWNWeb Security Administrator Registration
The use of the CROWNWeb computer software system, for entering and maintaining facility and patient information, is required by the new Conditions for Coverage.   To access CROWNWeb, each facility must designate at least one CROWNWeb Security Administrator.   A backup Security Adminstrator is also strongly recommended.   Each Security Administrator must register with the CROWNWeb Authentication Service (CAS), also known as the QualityNet Identity Provisioning System (QIPS).   The registration form can be found at http://www.qualitynet.org/crown/registration_form.pdf.   Instructions for completing the form are at http://www.nwrenalnetwork.org/CfC/CWSecurityAdminReg.pdf.One copy of the form for each Security Administrator must be typed without corrections, signed, notarized, and mailed to the Network using USPS certified mail, return receipt requested.   The form cannot be faxed.   Please mail your forms as soon as possible, as both the Network and the CROWNWeb QIPS team must process the form to authorize your Security Administrator before they can use CROWNWeb.   Your Security Administrator, once authorized, will then register the people at your facility who will enter and maintain the data.   We also ask that you fax to us the name(s) of your Security Administrator(s), before December 15.

Class 1 Recall of Insulin Pump Battery Caps
The FDA has announced a Class I recall of the caps which hold the batteries on Animas 2020, IR1200, and IR1250 Insulin Pumps, and on the OneTouch Ping Glucose Management System.   The defective battery caps may not hold the battery tightly enough to maintain electrical contact, which can lead to irregular insulin dosage.   Replacement caps and instructions have been mailed to consumers, but healthcare providers are asked to assure that the caps have been replaced.   For details see http://www.fda.gov/medwatch/safety/2008/safety08.htm#batterycap.   Class I recalls involve situations that may result in serious injury or death.   There are earlier articles on Insulin-Related Recalls.TOP OF PAGE     BOTTOM OF PAGE

Waivers and Extensions on Conditions for Coverage
CMS has released a document on applying for waivers and extensions to the new Conditions for Coverage.   The new Conditions went into effect October 14, 2008, but there are separate deadlines for isolation rooms, fire code compliance, certification of Patient Care Technicians, and electronic submission of data.   In certain circumstances, if patient safety and the quality of care are not compromised, a facility may apply for a waiver or an extension of time to comply with new conditions on isolation rooms, fire codes, water tank shape, Medical Director qualifications, single-use vials, or comprehensive patient assessments.   For complete details see http://www.nwrenalnetwork.org/CfC/WaiversExtensions.pdf.   There are earlier articles on Conditions for Coverage.TOP OF PAGE     BOTTOM OF PAGE
November 21, 2008 The quality measures on CMS's Dialysis Facility Compare (DFC) website, at http://www.medicare.gov/Dialysis/Home.asp, have been modified.   They now show the percentage of patients at each facility with hemoglobin lower than 10 g/dL and the percentage of patients with hemoglobin higher than 12 g/dL, rather than the percentage of patients with hemoglobin of 11 g/dL or more.   The acceptable range for survival "as expected" has also been reduced, so that now some Network facilities show survival "worse than expected" or "better than expected."   These changes are introduced in the November 20 CMS press release at http://www.cms.hhs.gov/apps/media/press_releases.asp.   Note that the internet address for DFC given in the last paragraph of this press release does not work; use http://www.medicare.gov/Dialysis/Home.asp instead.   There are earlier articles on DFC.

Conditions for Coverage Impact of CROWNWeb - Important Training Windows

Conditions for Coverage Implementation through Survey and Certification Open Door Forum

September 9, 2008 On September 10, 2008 CMS will hold an Open Door Forum to announce the convening of a Special Needs Plan Chronic Condition Panel.   The Panel will determine the conditions that meet the definition of severe or disabling chronic conditions in accordance with the Medicare Improvements for Patients and Providers Act of 2008, which amends the Social Security Act's definition of a "severe or disabling chronic conditions specialized needs individual" to include those "who have one or more comorbid and medically complex chronic conditions that are substantially disabling or life threatening, have a high risk of hospitalization or other significant health outcomes, and require specialized delivery systems across domains of care."   CMS will hold a conference call on September 10, and accept written comments until October 8, 2008.   For more details and participation instructions, see http://www.cms.hhs.gov/OpenDoorForums/05_ODF_SpecialODF.asp.

New Conditions for Coverage on Patient Assessments

Warning on Diabetes Drug Byetta

June AVF Rate Increases to 62.7%
The Network AVF-in-use rate for June rose to 62.7%, from 61.9% in December 2007 and 62.0% in March 2008.   The Medical Review Board recently raised the Network goal for this rate to 66%, to match the 2009 CMS national goal.   An additional 8.7% of Network patients had a maturing AVF in place.   The incident-patient AVF-in-place rate for April-June 2008 fell to 36.0%, from 37.0% in the previous quarter.   As always, the Network's progress is posted at http://www.nwrenalnetwork.org/fist1st/fist1st.htm#progress.   There are earlier articles on Vascular Access.TOP OF PAGE     BOTTOM OF PAGE

2008 Dialysis Facility Reports have been mailed to Medical Directors and Facility Administrators at all Network dialysis providers.   Facilities have until September 7, 2008 to comment on their DFR, at https://secure.arborresearch.org/esrdmeasures, using login IDs and passwords printed on the cover of your DFR.   These comments will be included with your report when the DFRs are mailed to State Survey Agencies in September.   CMS will also review these comments before they post your facility information on the Dialysis Facility Compare website in November.   Note that the website address begins with "https:" rather than "http:", and does not include "www."   It would be a good idea to protect your password from unauthorized viewers.   There are earlier articles on DFRs.TOP OF PAGE     BOTTOM OF PAGE

CROWNWeb - Where to Find Information and Send Questions/Comments

Fistula First UpdatesThe Network prevalent-patient AVF rate climbed to 62.1% in May, up a fraction since December.   The Medical Review Board has raised the Network goal to 66%, to match CMS's goal.   As usual, our progress is reported at http://www.nwrenalnetwork.org/fist1st/fist1st.htm#progress.

Free RSN Seattle Regional Patient Lifestyle Meeting in Seattle Area August 17

July 3, 2008 The Network has been notified that a contamination of Morton Salt Pellets used by dialysis units to regenerate water softener has been identified.   One lot number that has been identified as contaminated is May19AHD.   At this time Morton is unaware of any other lot numbers that have been contaminated.   The contamination manifests itself as blue flecks in the white salt.   The blue can range in color from sky blue to royal blue.   The actual contaminant is Yellow Prussiate of Soda (YPS).   Morton recommends that all dialysis facilities utilizing Morton Salt Pellets check each lot for contaminants prior to use, and discard any lot containing blue flecks.   The effect of YPS on water safety is unknown.
