Boston Scientific Implantable Defibrillators Recall

Proposed Transplant Policy Changes

Plavix (Clopidogrel): New Boxed Warning
The FDA has announced that a new Boxed Warning has been added to the prescribing information for the anti-blood clotting medication Plavix (Clopidogrel).   The label includes a warning about reduced effectiveness in patients who are poor metabolizers of Plavix, information about tests to identify genetic differences in CYP2C19 function, and suggested alternative strategies for patients identified as poor Plavix metabolizers.   For details see http://www.fda.gov/Safety/MedWatch/SafetyInformation/SafetyAlertsforHumanMedicalProducts/ucm204256.htm.   There are earlier articles on Plavix.

Baxter HomeChoice & HomeChoice PRO PD Systems Class I Recall

LifeScan OneTouch SureStep Blood Glucose Test Strips Recall
The FDA has announced a recall of eight lots of LifeScan OneTouch SureStep Blood Glucose Test Strips, lot numbers 2969251, 2969481, 2969795, 2969798, 2982369, 2982566, 2983467, and 2998193.   Lot numbers  are located on the outer carton and test strip vial.   It is important that patients with recalled test strips continue to test their blood glucose.   Patients with access to a meter that does not use OneTouch SureStep Test Strips should use this other meter to test their blood glucose until replacement product from LifeScan arrives.   If an alternate meter is not available, patients may continue to test using the recalled OneTouch SureStep Test Strips.   However, if patients obtain results above 400 mg/dL, they should contact their healthcare professional for further instructions because their glucose may be significantly higher.   For full details see http://www.fda.gov/Safety/MedWatch/SafetyInformation/SafetyAlertsforHumanMedicalProducts/ucm202254.htm.   There are earlier articles on Insulin-Related Recalls.

FDA Review of Avandia (rosiglitazone)
The FDA has notified healthcare professionals and patients that it is reviewing a large, long-term clinical study on possible cardiovascular risks with the diabetes drug, Avandia (rosiglitazone), as well as published observational studies on the subject.   No new conclusions or recommendations about the use of rosiglitazone in the treatment of type 2 diabetes have been made at this time.    An updated assessment of rosiglitazone will be provided at a public meeting in July 2010.   The FDA recommends that healthcare professionals follow the drug label when prescribing rosiglitazone, including the Boxed Warning.    Patients should continue taking rosiglitazone unless told by their healthcare professional to stop.    Patients who are concerned about possible risks should talk to their healthcare professional.  For more information, see http://www.fda.gov/Safety/MedWatch/SafetyInformation/SafetyAlertsforHumanMedicalProducts/ucm201446.htm.

Five-Diamond Patient Safety Program

The Virginia and New England Networks have assembled a “5-Diamond Patient Safety Program” with the intention of building a patient safety culture in every dialysis unit, promoting patient safety values, creating an awareness of patient safety issues, and helping dialysis facilities learn more about specific areas of patient safety.   Their introductory press release is at http://www.esrdnet5.org/5diamondptsafety/pressrelease.pdf.   The original program included modules on Patient Safety Principles, Decreasing Patient-Provider Conflict, Emergency Preparedness, Infection Control, Influenza Vaccination, Medication Reconciliation, Missed Treatments, Sharps Safety, and Slips, Falls, and Trips.   They’ve recently added three new modules, on Health Literacy, Patient Self-Managed Care, and Stenosis Surveillance.   All twelve modules, and program details, can be found at http://www.esrdnet5.org/5Diamond.asp.   Each module includes all the tools and resources necessary to implement the patient safety concept.   The program is endorsed by ANNA and RPA.   We’ve also added a link from the QI page on our website, at http://www.nwrenalnetwork.org/QI/QI.htm#sfty.   There are earlier articles on Patient Safety, on Vascular Access, on Emergency Preparedness, and on Quality Improvement.

FDA Drug Safety Notice on ESAs
Erythropoiesis-Stimulating Agents (ESAs): Procrit, Epogen and Aranesp: Drug Safety Communication
The
FDA and Amgen have notified healthcare professionals and patients that all ESAs must be used under a REMS risk management program.   As part of the risk management program, a Medication Guide explaining the risks and benefits of ESAs must be provided to all patients receiving an ESA.   Under the ESA APPRISE Oncology program, Amgen will ensure that only those hospitals and healthcare professionals who have enrolled and completed training in the program will prescribe and dispense ESAs to patients with cancer.   Amgen is also required to oversee and monitor the program to ensure that hospitals and healthcare professionals are fully compliant with all aspects of the program.   FDA is requiring a REMS because studies show that ESAs can increase the risk of tumor growth and shorten survival in patients with cancer who use these products.   Studies also show that ESAs can increase the risk of heart attack, heart failure, stroke or blood clots in patients who use these drugs for other conditions.   Read the complete MedWatch 2010 Safety summary including links to the Drug Safety Communication and current Prescribing Information for these products, at http://www.fda.gov/Safety/MedWatch/SafetyInformation/SafetyAlertsforHumanMedicalProducts/ucm200391.htm.   There are earlier articles on EPO Use.

December 2009 AVF-in-Use Rate at 64.5%

Disaster Preparedness Information from FEMA

FEMA has prepared a website tabulating, for each state, contact information for local emergency preparedness agencies, and providing preparedness information and links on the types of disasters that are likely to occur in that state. The images from Haiti continue to illustrate the devastating impact of natural disasters.   The U.S. Department of Homeland Security (DHS) is working hard to support the international response effort to help our neighbors recover from this tragedy.

Emergency preparation may not be possible for every individual, or for every type of emergency, but we cannot overstate the importance of preparedness planning.   The U.S. federal government, along with many of our partners in the private sector, provides abundant resources for preparing your family for disasters.   We encourage you to utilize these resources and prepare your family to the fullest extent you can.  If you live in a region prone to earthquakes, you are probably aware of the risks they pose.   Knowing the risks within your community is the first step in evaluating the preparedness level of your family.   DHS encourages you to research the potential risks in your area, evaluate how you should best respond, and prepare your family accordingly.    Forethought and planning will help you make sound decisions during a crisis and keep your family safe.

Consider the following tips for preparing your family for emergencies:

· Visit the Federal Emergency Management Agency (FEMA) website and click any state to see the  common local risks and how to best respond.

· Find out what your community is doing to prepare for emergencies and volunteer.

· Visit the websites of organizations and government programs like Ready.gov, Citizen Corps, and the National Safety Council and utilize the resources they provide.

· Subscribe to the free Citizen Corps news email service for community preparedness news and updates.

· Visit the website of the Home Safety Council to learn how you can make your home safer for you and your family. 

· Take classes in lifesaving skills, such as CPR/AED and first aid, or in emergency response, such as CERT.

· Work with parent-teacher organizations to discuss how you can support their emergency plans and drills.   The Office of Safe and Drug Free Schools and Readiness and Emergency Management for Schools will have information on school emergency preparedness and response.

· Help children prepare for emergencies at their own pace and maturity level.

Involuntary Discharge Notification Requirements

Mikro and Haematokrit Centrifuge Recall

H1N1 Vaccine Status Letter from FDA

Rapamune/Sirolimus Prescribing Information Changes
The FDA has announced changes to the prescribing information for Rapamune/sirolimus, warning that a change in assay type, in laboratory, or in lab reference value can result in inappropriate dosage which can lead to graft rejection or toxic side effects.   The FDA advises that it is critical that the clinician caring for a patient on sirolimus maintain communication with their laboratory to determine whether the assay used for measuring sirolimus concentrations has been changed.   For details see http://www.fda.gov/Safety/MedWatch/SafetyInformation/SafetyAlertsforHumanMedicalProducts/ucm197059.htm.   There are earlier articles on Sirolimus/Rapamune.

National Influenza Vaccination Week January 10-15

Though the H1N1 “Swine” flu seems to have subsided for now, most dialysis patients are actually at greater risk for “normal” flu, which remains in season.   The CDC web page on National Influenza Vaccination Week, January 10-16, 2010, is at http://www.cdc.gov/flu/NIVW/index.htm.   It contains a number of resources to help develop a vaccination campaign.   In particular, Tuesday January 12 is set aside for vaccination of “people with chronic health conditions that put them at high risk of serious influenza-related complications.”   There are earlier articles on Immunization and on Pandemic Influenza.
