Lifeline and ReviveR Defibrillator Battery Pack Recall
The FDA has issued a Class I recall of the Defibtech DBP-2800 Battery Packs used in Lifeline and ReviveR
semi-automatic external defibrillators, because they may cause the AED to malfunction.   For details and recall instructions see

Claris Cipro Recall, Including Metronidazole and Ondansetron
Claris Lifesciences and the FDA have announced a recall of all lots of Ciprofloxacin Inj. USP 200 mg / 100 mL (NDC 36000-008-24), Ciprofloxacin Inj. USP 400 mg /
200 mL (NDC 36000-009-24), Metronidazole Inj. USP 500 mg/100 ml (NDC 36000-001-24), and Ondansetron in 5% Dextrose Inj. 32 mg / 50 mL (36000-014-06) manufactured and distributed by
Claris Lifesciences.   The recall is a precautionary measure against possible contamination due to packaging integrity.   Healthcare professionals should NOT use these products,
and should immediately remove them from their pharmacy inventories.   Find further recall instructions at

Dialysis Population Growth
The Network dialysis population grew 6.2% year over year in the January-March quarter, a full percentage point faster than the corresponding 2008-2009 growth rate.   Growth in 2009-2010
was about 5.5% in Washington and Alaska, 6.7% in Oregon and Idaho, and 8.5% in Montana.

March Network AVF Rates
The data is still preliminary, but the Network’s overall prevalent-patient AVF-in-use rate at the end of March 2010 was 65.0%, one percentage point higher than a year earlier, but still one point
shy of the 66% Network and CMS goal.   While the incident-patient AVF-in-place rate was up a few percentage points in the January-March quarter, at 40% it’s still only two thirds of the
Network goal, where it’s been since 2006.   We’ve updated our monthly and quarterly progress reports at

Revised Survey Guidance on Conditions for Coverage
We’ve updated six documents on our Conditions for Coverage webpage (http://www.nwrenalnetwork.org/CfC/CfC.htm) and our
Survey webpage (http://www.nwrenalnetwork.org/CfC/Survey.htm).   They include Interim Version 1.6 of the “Measures
Assessment Tool” (MAT), Version 1.4 of the list of “ESRD Facilities V-Tags & Identifiers,” Version 1.3 of the “Correlation Grid for Patient Assessment and Plan of Care and Timelines for same,” and
“Program Survey Guidance on PCT Certification,” along with its two attachments – “FAQ and V-Tags on PCT Certification,” and “Matrix for Documenting PCT Certifications.”   The PCT documents
update the April editions (see http://www.nwrenalnetwork.org/news_bulletins10B.htm#pct4j).

Class I Recall of Nihon Kohden and GE Responder Defibrillators
The earlier Cardiac Sciences Automated External Defibrillator Class I recall (see
http://www.nwrenalnetwork.org/news_bulletins10A.htm#aed2f) has been extended to include Nihon Kohden (NK)
and GE Responder models.   For details see

Patient Newsletter on Remaining Active
Volume 3 Issue 4 of the patient newsletter Network Consumer News has been mailed to facilities for distribution to patients.
